I work at McDonald's and once walked into the disabled toilets - which has a lock on the door - to make sure it was clean only to be met by three children (one of whom had obviously been playing with the lock) and a rather attractive mother with her pants down mid-job.
Her reaction could be summed up as AAAAAAAAAAAAAAAAAAAAAAAAH!
So yes, lock the door when you're peeing too.
