FWIW, this has been the most successful method for me.
It has to be done on a night when I know I can get 8+ hours of sleep. I'll tell myself when first going to bed that I will wake up after I have a dream, and remember it. This always causes me to awaken after one of my sleep cycles (usually about 9 minutes each) -- maybe I'll get 3 hours in before the first awakening. After I've had 4-6 hours of sleep, and awaken, I will get out of bed and stay up for 30-60 minutes. During this time, I'll usually have a couple of cigarettes, a chocolate bar, and something to drink (something with a little caffiene, like Coke, is probably the best thing). I'll usually also read DV or chat with other DV members via IM during this time. Any other activity that makes you think about lucid dreaming is probably just as good -- but I don't suggest watching TV or something else that would distract your mind from the task at hand.
Then I'll go back to bed. To get my body relaxed, I'll do the thing where I contract and relax all the muscles in my body. Then I'll concentrate on breathing deeply, while thinking "I am going to have a lucid dream" etc...
I've managed to get 1-3 LDs per week using this method, when I have the time to do it properly (and when I'm not too stressed out about other things).
