I think it's around that age (4 or 5) that we stop becoming the "center of the universe," so to speak. We become aware that other people and kids also have their own minds. That's when we become conscious of ourselves as separate, distinct entities as well. Anyway...
The "sudden" awareness of your nakedness in stark contrast to others may mean that you recently started to see a glaring difference between you and them, and it was something you felt ashamed of. It can be financial, material, physical or sexual. However, it seems that they didn't notice it. This might mean that it is something that nobody else noticed, and that only you noticed. Or maybe it's not a big deal, but you think it is.
You don't like your trainer in the everyday world, but how did you feel about her in the dream? Were you also ashamed that she saw you naked? You are right, it is doubtful that it was really the trainer. It might be someone else. It might even be an aspect of yourself that is responsible and knowledgeable. How you respond to and feel about a person in the dream is usually more important than the image, which is usually just used by the mind as a symbol to get a message through (trainer = responsible, knows a lot, experienced).
It *could* be referring to your attempt at gaining weight. Maybe you see everyone else as physically better, and you feel that lack that. But since your dream does not seem to show that anyone else noticed, it might be less serious than you think. Maybe others actually think that you are in a perfect weight or whatever.
