The figure of "death" is often portrayed as wearing a black coat or cape, and with a black hat, and is especially creepy in silhouette. I think here, you're being asked to open your mind about the subject of what happens when you die. He tells you, you can't know, because he's seeing you are close-minded about it, thinking that you *do* know! Heart attacks might not be literal, so much as saying your feelings are dead, you aren't feeling things you should be feeling. We all grow up intellectually first, and then develop emotionally. You need to allow yourself to develop that part of you, so your heart isn't dead.
I can remember when I was young and so much of what I thought I felt was just imitating what I was "supposed" to feel under certain circumstances, and I can remember when my grandmother died, and I knew exactly what I felt, maybe fully for the first time, really. The feelings were real, not programmed, not me acting out what I was supposed to feel. It's kind of like the story of Pinocchio where he's a puppet first, and then he comes to life as a real boy. We all have to do that, and it's our emotional self that brings us that difference, so we're not just programmed like androids or controlled like puppets. Death also symbolizes change, and letting this side of yourself develop will be a major change, but it is a necessary one.
