It's a movie with poor logic
The "levels" idea in Inception is a concept of dreaming created by the movie.
Inception used the idea of "levels" to show deeper and deeper aspects of conscious and repressed memory (in the case of Leonardo Di Caprio's character). These levels were accessed by various methods, via an elevator or the device used to put the agents into the target's dream. Levels are a fictional idea created by the movie for the sake of storytelling and has nothing to do with dream science. To answer your other question though, it may have been tough to reach the third level because of how deep into the unconscious it is.
On a brighter note though, just because Inception invented the idea of leveling, it doesn't mean that we can't access deeper parts of our unconscious through lucid dreaming. For, that's what dreaming is all about ! There are no limits to self-discovery and self-understanding through Lucid Dreaming. It's important to remember that dreaming is very personal to the individual, and how one confronts and solves problems presented in a lucid dream varies from person to person. The movie may have provided it's own way of explaining how to venture into the unconscious, but it's not how YOU will. With intention, there are unlimited ways to creatively travel further and further to unknown aspects of the self.
The idea was, they were creating a dream within a dream, and to do this, they had to acually fall asleep and enter another dream within the dream state. In reality, the dream within a dream scenario is more likely explained by the common occurance known as false awakenings, thus it is all the same dream state.
False awakening to me is more like a forceful reset of your dream level. Like you lost connection with your dream, but it is just a 1 second black out, you barely get a hold of it again and the level got reset.
