I don't know how to make those cool achievement box thingies, but here are a few ideas...
Leeroy Jenkins - Rush into a room full of monsters and start slaughtering them (optional: scream your name as you run in)
Agent Smith - Turn a DC into yourself
Uber Micro - Lead your massive army in national/global/galactic/intergalactic conquest, RTS-style
Indiana Jones - Successfully escape a giant rolling boulder
There Is No Spoon - Bend a spoon with your mind
