I think you are supposed to repeat the mantra in your head. I'm not sure about the moving, but I think it should be ok if you odn't give it much attention.

When you say your mantra out loud it will be more powerful, but it might keep you awake. You could start with saying it out loud a couple of times and then continue in your head. The essential thing of MILD is to really focus on your mantra. If you worry about moving, you will not be completely focused on your mantra. So, move or don't move, at least don't pay any attention to it.
I do it while I get up to go to the bathroom during a 3 minute WBTB. :) Used to almost always work for me that way after I gained some experience.
