Prospective Memory Log

I made this for my own use, but I thought that someone else might want to use it. It's just a simple log for the type of prospective memory exercise mentioned in EWLD, in the MILD section.

If you print this on one page, you can do a sort of half-page folded booklet, which is handy. It has seven entries for each of seven days, where you can record whether an event occured, and whether you managed to notice it and perform the RC. There's a table at the end for recording your totals, and to graph your results, if you wish.

There's one blank template, and one filled with targets (not the ones from EWLD), in pdf, odt and doc formats. If there's an issue, holler.

It's a .rar, not a .zip. I had to rename it to get the forum to accept it.
