How To Fly Successfully?
I cannot fly at all in my dreams. I can only jump really far really slowly (and it makes my stomach feel funny, like on a roller coaster).
I try to fly and I end up levitating half an inch off the ground or bellyflopping hard onto the floor.
The only times I did "fly", it didn't feel legit to me. One time I was with my mom, who could have been the one truly flying, and I was just hanging on. The other time I was either shot into the air like a rocket or hovering in one spot for one second.
(Admittedly, I did fly our family car over San Fransisco, but that wasn't the type of flight I am trying to achieve)

Could be a problem with belief, you gotta believe you can fly, haven't you ever seen Peter Pan? And if that doesn't work there are numerous other things you can try for instance: try drinking a magic flight potion, or spawn a jet-pack, or grow your own pair of wings straight out of your back, just to name a few options.
Anyways, good luck with your dreaming endeavors.

As Kraom said, sounds like you have a little niggling doubt in the back of your head every time you are about to fly. Use any sort of technique that would make it logical for you to be able to fly in the dream. For instance, the techniques above, a potion, wings, jet-pack... If you find a way to make it logical to yourself and your subconscious, there will be no doubt which means you will fly :)

I remember when I first started, I could not fly, all I could do was levitate one inch off the ground, in upright position and kind of glide on one inch of air for a while. Added to that I had low enough awareness that I thought in my dreams "I can float. That means I must be dreaming! But I could float, even if I were awake." And then I would just continue to float and not do anything. Upon waking up, I would discover that, no, I most definitely could not float when awake.
It was pretty pathetic. I don't remember how long the pathetic LDs lasted for me. Yes, I bet it had to do with doubt and issues with my expectations.
What I do remember is that then there came a night when I became lucid because I became aware that my home in my dream was not actually like my waking home at all. I remember it was one of the most vivid dreams ever. I opened a window with just my mind. I flew out of it, and soared out into the sky and then into outer space. That was about 20 years ago during my previous LD practice experience, and yet I still remember that dream vividly. My latest LD recently was not quite as exhilarating, but it included some high quality flying at rooftop level, very pleasant.
You too can do this. You have to believe that, expect it, and it will happen, and it will be fantastic. :)
