I can't say that I've gotten panic attacks while trying to sleep. Why do you fear death, emptiness and loniliness? I mean, yeah, they aren't great things, but the most important thing for you to do is to assure yourself that you have no reason to fear them.
If you're doing death defying stunts, then stop. If you feel empty, then go out and do something beneficial to others and fulfilling. If you're lonely, go out and meet new people, hang with friends/family, or if you just want company, then get a pet. Be more social
Perhaps you have problems with abandonement?
I did have the same vision everytime too. I visioned myself falling upwards, into this void, where i could see stars and such, so I'm assuming it was space that I was seeing, and I could see my arms stretch out in front of me like i was reaching to get of hold of something, anything, just so i wouldnt feel alone. then id wake up in a panic. I dont quite get it. Is this a dream, or something you are envisioning while trying to go to bed because if this "vision" is one of the causes to your panic attacks, then stop thinking about it. Visualize calm, peaceful, and in your case, friendly/populated places. I guess one would be the beach.
