Since you used to work as an insurance salesman, I think that is a literal memory not a symbolic standing in for something.

However, if it were not, what I think of when I think insurance is just in case of trouble, helping people be prepared for worse case scenarios. The fact that this is at a car dealership: cars help people go places, so movement change. So maybe if this is metaphorical, it was your responsibility in this dream to help people prepare for the unexpected bad changes in life. Afterlife comes after death which is the ultimate unexpected bad change. In your dream you are not prepared, not ready to handle it without help. You seek out the help of an older relative, maybe because she has more experience in life with changes, maybe because you know she is prepared for the eventuality of death or another unexpected bad change that you feel you are inadequate in preparing for. Was the stroke unexpected, or has your grandmother had health issues? Is she the oldest, most frail member of your family, most likely to die? I wonder also whether this dream is about your uncertainty whether there is an afterlife, perhaps you wish you could believe in it as sort of an insurance policy to be prepared for the eventuality that there is an afterlife, and you feel that you need help in learning to believe in an afterlife from an older relative?

MOVING AHEAD.
You wanted to move ahead, and gain insurance in this - if anything goes wrong or you lose, you are backed up. Maybe in other words, you don't lose, you gain. You are in-sure, or sure in you.
So you trained yourself for this, or wanted to, but could not accomplish the task or what was needed.
You went to find someone who could help you. That sounds like you realized you couldn't do it alone.

WITH OTHERS
Now you go down a new path, that has many others on it. Perhaps your task is to connect or do with others. To get moving with people? Or maybe you saw, after all, they give you peace that you wanted to get alone? Perhaps this selfish part of you had to die, and now this is your life after.
Now maybe you see you are not close with people, represented by your maternal grandmother you are not close to.
And you get on your back because you are incapacitated in this area. And this side of you was helping you move forward in your training maybe to be close to people, to which there was no specific outcome yet.

MEANING
Perhaps you saw that to move ahead, you need others and this will give you the peace you want.

That she had a stroke might be of special timing, a meaningful co-incidence, if it ties in with the dream.
If you are starting to get close to others at the exact time a family member you were not close to may be one step closer to the afterlife.
Then again, it may be more special or connected - as you'd think it may be:.
You get on your back because you are incapacitated is just like a stroke victim. And she, who had a stroke, is now helping you with movement as she might herself. Seems more than coincidence.
But the meaning remains the same.
