Dream interpretation is really best left to the dreamer, in my belief. But, there are things that other people can do to try and help you figure things out. Situations and objects in dreams will mean different things depending on who is dreaming them.
In his book, Exploring the World of Lucid Dreaming, Stephen LaBerge talks about the idea of schemas. Basically, this theory says that dreams are created by what a person expects to happen in certain situations based on past experience. Often, a dream starts with just a random setting or object and then your expectations about that setting or object create the "plot" of the dream.
In your case, it is possible that you were focused on your tongue for some reason in the dream, and when you focused on it, you found it had become a penis. Now, it is possible you had a recent experience that brought this imagery to mind, or that it was actually a fairly random thing your brain came up with. The part about the dream that I believe is of real interest is how you reacted to your situation and rationalized it. You thought, "why would my tongue become a penis?" and your subconscious came up with the solution that you'd had surgery. Why would you have surgery? Because your boyfriend wanted it. The fact that you came up with that answer could possibly indicate that you fear that people won't like you for who you are. You might feel like you have to change for other people a lot of the time. Once again, only you would know, but this is a possible interpretation. The concerns about your BF being gay probably stems from your dream brain concluding that if he wanted your tongue to be a penis, that would raise suspicions about his sexuality. I doubt that it means you worry about his sexuality in real life. It was just a way of your brain explaining the situation you were in. It is interesting that you felt defensive of him in the dream rather than angry. This might indicate that you are sensitive to how others feel about him. Your feelings of guilt and the need to hide your surgery also may indicate you are worried about what others think of your relationship.
Once again, keep in mind that you know best what your dream means to you. In my opinion, your reactions to your deformation in your dream are more important than just the image of the penis tongue. Our subconscious does all kind of strange messed up things with the appearance of our physical bodies in dreams. A surefire way to have something fucked up happen to your dream body is to stare in a mirror.
