The heart problem isn't coming from the dreams or the lucidity, but the anxiety. Your lucidity could actually help with your anxiety because you know that since you're dreaming those germs aren't real. So, you can face your fear and come out of the dream feeling accomplished rather than anxious.
That said, if you want to wake up, just fall asleep in the dream and wait until you wake up. Try to imagine yourself in bed. When you do wake up, do a reality check. If you're still dreaming, fall asleep in the dream again and wait some more. Be patient! If you experience 100 false awakenings, whatever. Just wait it out. You're not dead or dying or in a coma, you're just dreaming. You're safe!
