Holy crap, that's from Dan Brown's book, Angels and Demons. Spoiler for What Happens Next: The "Earth" symbol shows up branded to the man's chest after he turns up with a windpipe crammed full of dirt, no? Been a while, but I think the next one is "Air," and the guy dies in public from two puncture wounds on either lung. CPR only results in a mist of blood. Third one is fire, the guy shows up in a church, strapped to the giant incense pendulum thingy, and cannot escape as the church goes up in flames. The last guy, water, is drowned in a fountain, along with Peter Langdon, I believe.
