DV Academy needs help from someone with technical know-how!

The DV Academy is coming along nicely but an aspect that we need help with requires a bit of technical know-how and I was hoping that some of you might be able to help!

Two applications need to be hosted- a student application and a professor application. These applications have to be fillable by DV members and then somehow submitted to the people in charge of the DV Academy. If this can be done through the forums, that would be awesome. I have no idea what is possible, but you guys probably do

Originally I created a student application in PDF format. However, fillable PDF forms can't be saved into PDF format and then uploaded, so it has to be done some other way. I know that HTML forms can be set up, but I have no idea how you host them or how you establish who it gets submitted to. Here is that PDF in html format... unfortunately I just had to use www.phpform.org and didn't actually know how to do the technical stuff. If you can help, please let me know We want to get the Academy running asap Thanks!!!

Spoiler for HTML for a form? Does this work?:

A professor application hasn't been created yet, since I didn't want to put in a lot of effort and find out the format wouldn't work. If you are able to help, please let me know and I will get you the information for the form ASAP!

What error were you getting when trying to upload a PDF with forms? The uploaded shouldn't be checking what's in the pdf, just the extension? Or do you mean that you don't have Adobe Pro? If that's the only problem, any Office suite other than Microsoft office can export PDFs with forms.

I was running a trial of Adobe Pro which is how I made that PDF application.

The problem isn't in that application itself or uploading it, but the fact that you can't save one of those PDF forms once it's been filled out. It won't keep the information you've entered, it'll only save the form itself So it's not so much getting the PDF out to applicants, but getting their completed applications to save their responses and then have a means of uploading it for the DV Acad profs to review.

All I've got is Microsoft Office, so I wouldn't know know anything about that. Is there a way to export them that everyone on DV, regardless of what OS and etc. they're running, can use to save a filled-out form to then upload?

What, exactly, do you want to do with the info entered into the student application?

If it's just simply having the info readable, why not just create a new sub-forum, with a template post, and let people create a new thread, using the template post as a template?

Does the student info need to be confidential? (there's next to no personal info required)

It's very possible for the info on the form to be crafted into a forum post, and posted on the forum
how useful that will be depends on what you're going to do with the info.

but again, it's probably easier all round for people to just use a template post (stickied) in a new sub-forum

Heck, that's what I said too.

But Shift reckons it would get too messy or something.

      Meh...

The information will just be used to match up applicants to professors and courses. It doesn't necessarily need to be confidential, though there are a few questions that I know people can be uncomfortable answering from the adoption program. I agree just posting the info will serve the purpose, but it's an epic bummer if I spent all that time on the PDF/html form for it to end up like this:

      Spoiler for App:

(I got too lazy with the brackets).

Someone also mentioned how clean and 'official' or serious it looked in PDF format and I agree. If at all possible I want it to be clean and presentable and serious. If there is any way that I can put in some more effort to improve the quality a little bit, I'd be happy. Unfortunately I don't have the knowledge, but if someone can even just direct me I'd be willing to work harder to pull it off. It will also be way easier to just open a bunch of PDFs and compare answers than it will be to look at all those responses in the sloppy thread format So yes, as a last resort I guess this wouldn't be so bad, but I'd really like to see it in a more presentable format.

Not to mention, I think if I ask for another subforum they'll murder me And I've no interest in being PMed applications

      According to Adobe,
Fill and Submit Forms via E-mail: This type of PDF form contains data fields and provides a Submit button. This moves the data off the user’s desktop and places it as an attachment in an e-mail message with the recipient’s e-mail address already in place. When the recipient receives the attachment, the form data can be collected and managed further.
