After I've got relaxed, i lie still on my back and start to count my breaths, saying "1 i'm dreaming", "2 i'm dreaming", "3 i'm dreaming" etc, i keep doing this until the hypnagogic imagery starts to become more concrete images, like little mini movies. About a minute or so after these start my field of vision begins to widen, and I try to reach through into the dreamscene (just one arm at first) This usually takes a couple of attempts, but then once i can move my dream arm into the dreamscene I then step into it with the rest of my dream body.
It took me a lot practice before i could do this reliably though, my best advice is just stick with it. Try to be aware of your thoughts, and any sensations you feel and notice anything that seems different and you'll get there soon enough.
