I would say that sensory input from the body is amplified in dream states.
A small pain in my stomach was agony in a dream, and I though I had been stabbed.
Upon waking, I had a mild stomache ache
Last night in a dream I was walking past a building when I heard a massive explosion. It shook me so violently that I thought I had been blown to bits.
When I woke up, my ear that has a slight infection was making a slight popping noise as it cleared wax.
So yes, we do feel pain in dreams, and it is often magnified many times.
