Is this a good layout for a dream journal/ Whats your DJ like?

Whats your Dream Journal like? I started a new one and this is the layout ( not in order...)

      Date:1-10
      Vividness:1-10
      Awareness (Mainly for LDs):
      Duration:
      Technique: MILD,DILD,WILD,FILD,ETC...
      Overall Dream Rating:1-5

      Description:

Picture (Whenever I feel like it...):

It so far has a Index, Reality Checks page, and a few more Im working on like tutorials in my DJ for induction techniques..

So whats your like and is this a good start?

My old one was a small notepad, which I rarely wrote in..

Whatever you feel comfortable with is good.
Mine is simple actually, I just write the date (for word) then a short feeling I had during the dream and then post the different dreams, separate them with a "Back to sleep" and done.
