In order for the hand reality check to work, you have to subconsciously expect the finger to go through the palm. Sometimes though, your subconscious expectation will be different from your conscious expectation, which leads to an unexpected result. For this reason, it's good to have a second reality handy in case the first one fails.

Don't be afraid to switch up your reality checks if they're failing more than you're comfortable with.
