I love the idea of the Academy courses! I think this will definitely help keep me interested in getting into LDing 
Here's my basics,
Reality Checks:
When dream sign occurs
- Location
- Memory
- Hands
- Awareness of surroundings
Dream Signs:
- Whenever I'm not aware
- Girlfriend / Roommates
- My house
- Video games or a gaming type of feel in the dream
- Any bizarre events or unfamiliar locations (during the day I imagine them or exaggerate oddities for practice)
Short-Term Goals:
- Four LDs a week
- Manipulate an object (size, color, etc.)
- Change the dream scene
Long-Term Goals:
- One LD a day
- Develop a dream HQ that will be a personal dream practice playground
- Get proficient with superpowers (telekinesis, flying, magic, etc.)
Lucid/Dream Recall History:
- A few short lucid dreams over the past few years since learning about it
- Since this year after wanting to get back into it I have had 2
- As of now I can only remember maybe 5 dreams per week
Current Technique:
- ADA / SAT
- Meditation
Typical sleep schedule: 11 PM - 7 AM
Why I want to lucid dream:
- To improve on aspects of my waking life (creativity, awareness, and memory)
- Inspiration for ideas or for practicing certain skills
- and for fun, duh 
Routine:
- Before bed I look over my recent journal entries
- I review the events of the past day from right before bed all the way back to morning to help improve my memory and practice how it would be to remember dream events
- Tell myself I will remember my dreams and be lucid in them as I fall asleep
