I'll be about holding someone or about to engage in "physical interaction" with someone, and all the sudden I'm just holding a lifeless object? For example, the most recent one was I was hugging someone in my last LD, and I looked back at her once we were done hugging and I found myself staring at a saxophone

Wow that's rather awkward I haven't experienced it myself but to prevent it you could try dedicating more attention to the person/thing you are holding and its features.

I have had all sorts of things like this happen. One of the more disturbing things was when the top half of a person disappeared. There was no blood, and the lower half was still walking around blindly. It really killed the mood.

Creepy. I would be wondering if there's any symbolism to it. Like, how that person or whatever she represented to you is similar to a saxophone. Or maybe it has something to do with "objectifying" DCs. I don't know, but I wouldn't be surprised if there's meaning to it.
